If you've been told you will "get a case manager" and aren't sure what that means, you're not alone.
Case management is a free-standing service that connects you to a single person whose job is to help you get and keep the mental health care you need, like appointments, medication, housing, benefits- all of it.

What a Case Manager Actually Does
A case manager (Alliance now calls this role a care coordinator, though "case management" is still the name of the service) is not your therapist. Their job is coordination, not counseling.
That means they:
Build a plan with you — your goals, not a generic checklist, written together at your first visit
Connect you to services — therapy, psychiatric care, housing help, benefits, transportation
Follow up — checking that a referral actually turned into an appointment, not just handing you a phone number
Step in during a crisis — a crisis plan is part of your intake from day one, so there's already a plan in place if things get hard
Stick with you — as of 2026, Alliance assigns one care coordinator per case, so you're not re-explaining your situation to a new person every few months
Who This Is For
Case management serves children, adolescents, and adults already enrolled in Alliance services — it's not a separate program you apply to on its own; it's layered on top of the care you're already getting.
It's most useful if you're managing more than one thing at once: a mental health condition and a housing question, a medication routine and a benefits application, a hospital discharge and the follow-up care that has to happen after. Targeted Case Management is a specific TennCare category within this for people who meet added eligibility criteria — your care team can tell you if that applies to you.
Case management isn't an extra Alliance offers when it has room. It's one of nine services required of every federally certified behavioral health clinic — which Alliance is. It's core to how care here works, not an add-on.

How Case Management Works at Alliance
Case management isn't housed in one building; our care coordinators are embedded across every setting Alliance serves people in: outpatient clinics, the Outpatient Mental Health program, team-based programs for more serious needs, the Jericho Project and housing services, and Tennessee's first-episode-psychosis program for young people.
At your first face-to-face visit, your care coordinator completes an assessment and writes a person-centered plan with you. That plan gets revisited regularly, and a crisis plan is part of it from the start, so if you ever need it, it already exists instead of being built from scratch in a hard moment.
Paying for Case Management
Case management isn't billed to you as a separate charge. TennCare funds it directly as a core behavioral health service, and it's included in the care Alliance provides regardless of your insurance.
If you're a TennCare member managing a behavioral health condition alongside another health issue — frequent ER visits, trouble keeping up with medications or appointments, or a recent psychiatric hospital stay — you may also qualify for Tennessee Health Link, a TennCare-funded care-coordination program that adds monthly check-ins and closer coordination with your medical providers on top of standard case management.

Is a case manager the same as my therapist?
No. Your therapist works with you on the clinical, emotional side of treatment. Your case manager (care coordinator) works on everything around that — appointments, referrals, benefits, housing, follow-up — so your therapist's time stays focused on therapy.
Do I need a specific diagnosis to get a case manager?
There's no single diagnosis requirement. Case management is available to children, adolescents, and adults already enrolled in Alliance services; your care team determines what level of coordination fits your situation, including whether the more intensive Targeted Case Management TennCare category applies.
