TN Health Link

Managing your mental health shouldn't feel like you're doing it alone. That's exactly why we offer Tennessee Health Link. TN Health Link is a special program that gives you a dedicated care coordinator who actually knows your name, your story, and what matters most to you.

What Makes Tennessee Health Link Different?

You know that feeling when you're trying to juggle doctor appointments, medications, and life. And something always seems to fall through the cracks? We get it. That's the exact problem Tennessee Health Link was designed to solve.

Here's what you get:

  • Your own personal care coordinator (yes, really – just for you)

  • Help connecting all your doctors so they're actually talking to each other

  • Someone to call when you're feeling overwhelmed or confused about your treatment

  • Assistance getting to appointments and accessing community resources

  • Support for your whole family, not just you

It's like having a GPS for your healthcare journey. No more getting lost in the system.

Who Can Join Tennessee Health Link?

If you have TennCare and you're dealing with:

  • Depression, bipolar disorder, or schizophrenia

  • Frequent trips to the emergency room for mental health concerns

  • Trouble keeping up with appointments or medications

  • Challenges managing both your mental and physical health

  • Recent hospital stays for psychiatric care

You might qualify for this program. And there's no waiting list and no extra cost to you.

What Your Care Coordinator Actually Does

They become your healthcare advocate. When you can't get through to your doctor's office, they make the call. When insurance is being complicated, they handle it. When you need a specialist but don't know where to start, they find one.

They help you stay on track. Missed an appointment? They'll help reschedule. Having trouble with side effects? They'll connect you with your doctor. Feeling like giving up? They'll remind you why you started this journey.

They see the big picture. While your psychiatrist focuses on your mental health and your primary doctor handles your physical health, your care coordinator makes sure everyone's working together. No more repeating your story five different times to five different people.

Your Questions, Answered

"Will this cost me anything?" Nope. If you have TennCare, Tennessee Health Link is covered completely. No copays, no surprise bills.

"How often will I hear from my care coordinator?" That depends on what you need. Some people check in weekly, others monthly. During tough times, it might be more. During good times, it might be less. It's totally up to you.

"What if I don't like my care coordinator?" You can switch. This is your healthcare, your choice. We want you to feel comfortable and supported.

"How long can I stay in the program?" As long as you need it and qualify for TennCare. There's no graduation date or time limit.
