WHAT TO BRING WITH YOU
Please arrive for your appointment at least 15 minutes early.
Be sure to bring your photo I.D., insurance card, and if applicable,
a list of current medications and diagnoses. If you are uninsured, please
also bring proof of income, proof of residency, and proof of citizenship.
INSURANCE INFORMATION
We accept a variety of insurance plans including Medicare and TennCare.
We also participate in the Mental Health Safety Net Program, which is
designed to offer help for people who may not
be otherwise able to afford services.
If you do not have insurance, you may qualify for a payment plan
based on a sliding fee schedule. No one will be denied
access to services due to inability to pay.
