Medical Records Requests

 

If you or a family member has received services at Alliance Healthcare Services and need a copy of medical records or a completed document, our Medical Records Department is here to help.

To request your medical records, contact the Medical Records Department directly

Email: medicalrecords@alliance-hs.org​

 

A few things to know:

- All requests — including those addressed to a specific clinician, therapist,  or care coordinator — should be directed to the Medical Records Department,  not to the individual provider.

- Emailed requests will be forwarded to Medical Records staff for processing.  (We are currently unable to verify identity via email, so a staff member may follow up to complete verification.)
